Long-term care through local support in rural areas

What is Rural Care?

RuralCare is a European social services innovation project that designs, implements, and evaluates a person-centered model of integrated long-term care adapted to rural areas. It seeks to enable people with dependency, disabilities, or chronic illnesses to remain in their homes, developing their life plans with personalized support, coordinated between social and health services. It is based on the active participation of users and collaboration between administrations and third sector entities.

Funded by European funds and led by the Social Services Department, Fundación Personas was the implementing entity. This consortium is completed by the Provincial Council of Valladolid, the University of Valladolid, Fundación Personas, and the ESN (European Social Network).

*Other collaborators: IMSERSO, Regional Ministry of Health, local councils in the area, and the Network of Provincial Federations of Retired Persons and Pensioners of Castile and León.

Who is it aimed at?

  • People, preferably elderly, who are dependent, disabled, or chronically ill and reside in their usual place of residence, whether owned, rented, or assigned.
  • Families and caregivers of the people participating in the project, offering them the support they need to perform their duties properly and balance this with their daily lives.

What activities does it include?

  • Design and implementation of a person-centered care model in rural settings.
  • Segmentation of households at risk and development of individualized support plans.
  • Coordination between social and health services for integrated care.
  • Training of professionals and strengthening of the role of coordinators and personal assistants.
  • Model evaluation and feasibility and sustainability analysis.
