Rural Care

It is an innovative project that offers people with dependency, disability or chronic illness the support they need so they can still live at home, life their life as normal, carry out day-to-day activities, become involved in the community, health care and, broadly speaking, any activities that make their lives more meaningful.
Acknowledging the person’s dignity, trying to make them as autonomous as possible, respecting their preferences and wishes, the capacity to adapt the support provided to the needs that may arise at any given moment and to help them become involved in the community are the aspects at the heart of any actions drawn up.
Who is it for?
- People, preferably the elderly, dependent, disabled or those suffering from chronically illnesses, who live in their own, rented or rented home.
- Families and carers of the people taking part in the project, offering them the support they need to be able to live in the best way possible, making it compatible with their day-to-day lives.
What is the aim?
To provide the social assistance, health care and opportunities to become involved in the community so people can safely live in their own homes with dignity and a good quality of life.
Support the family and other carers, to care for the person, finding a balance between their personal, social and working life while looking after their own health.
Drive regional dynamisation, with a view to fostering rural environments that facilitate people’s autonomy, encourage local employment, community involvement and social cohesion.
How is it funded?
The project and the services it offers are financed through the financial contribution of the European Union, support from the Dependency System as well as contributions from the participants depending on their economic capacity.

- A professional public case manager
- A professional case manager with mediation and dynamisation activities.
- A professional of reference at home and in the community.
- Personal assistance service.
- Home healthcare, scheduled and emergency care depending on the service portfolio.
- Home accessibility assessment and, where appropriate, Adapt home + technical aids + basic and advanced telecare.
- Support programme for non-professional carers.
- Personalised community participation programme.
- Providing the professionals with training.

- Tierra de Campos Sur: CEAS de Medina de Rioseco
What entities are involved?
It is a European Union funded project led by the Social Services.
The consortium comprises the Provincial Council of Valladolid, the University of Valladolid, Fundación Personas and ESN (European Social Network)
Other collaborators: IMSERSO, Regional Ministry of Health, Town Councils in the area and the Network of Provincial Federations of Retired and Pensioners of Castile and León.
