End-of-life care at home
What is the INTECUM Project?
INTECUM means “for you and with you”. This motto captures the spirit of the project: to be by the side of the person and their family in a particularly sensitive vital moment, offering accompaniment, support, and care with respect and closeness.
This is an initiative of the Junta de Castilla y León that allows people with advanced or terminal illness to remain in their homes, receiving the necessary support and maintaining their life project according to their wishes and preferences.
The care model is comprehensive and personalized, covering the physical, emotional, and social dimensions of the person, and also taking into account the needs of the family and caregivers.
Who is it aimed at?
The project is aimed at people included in the Integrated Care Process of Palliative Care of Sacyl who present:
- Incurable, advanced, and progressive disease.
- Limited life prognosis, with little response to specific treatments.
- Evolution with frequent crises and high demand for resources.
- Great emotional and family impact that affects the caregiving environment.
Since its launch, nearly 1000 people have been treated in the provinces of Valladolid, Zamora, Segovia, and Palencia
What services and support does it offer?
The program combines a comprehensive care model with practical support that allows the person to remain in their home, in conditions of quality and safety. The main actions are:
- Training and support for caregivers: training and accompaniment for professionals and non-professional caregivers to improve care and alleviate the family burden.
- Social and community participation: development of activities that allow maintaining bonds, relationships, and what is important for each person in their environment.
- Choice and guidance of the life project: the person defines, with the appropriate support, how they want to live this process and what they need to do it in their own home.
- Personalized home care: support for daily activities, focused on quality of life and staying at home.
- Prevention and socio-health coordination: comprehensive response to physical, psycho-emotional, and social needs, thanks to the joint work of social and health services.
- Home assessment and technical support: analysis of accessibility and provision of adaptations and technical aids that facilitate daily life.
- Personal and health care: support adjusted to each situation, including scheduled care.
- Accompanying in the final stage: positive support and active presence for both the person and their family, throughout the process.
- Support in grief: emotional and social accompaniment to the person’s environment after the loss.
Procedure for joining the program
Entry into the program must be managed through the social worker of the Health Center corresponding to the locality of residence.
Institutional collaboration
The project is promoted by the Ministry of Family and Equal Opportunities, through the Management of Social Services of Castilla y León, in coordination with Sacyl and in collaboration with social entities of the Third Sector, including Fundación Personas.
