It seeks to enable people with dependency, disabilities, or chronic illnesses to remain in their homes, developing their life plans with personalized support, coordinated between social and health services. It is based on the active participation of users and collaboration between administrations and third sector entities.  <\/p><p>Funded by European funds and led by the Social Services Department, Fundaci\u00f3n Personas was the implementing entity.
