This motto captures the spirit of the project: to be by the side of the person and their family in a particularly sensitive vital moment, offering accompaniment, support, and care with respect and closeness. <\/p><p>This is an initiative of the Junta de Castilla y Le\u00f3n that allows people with advanced or terminal illness to remain in their homes, receiving the necessary support and maintaining their life project according to their wishes and preferences.<\/p><p>The care model is comprehensive and personalized, covering the physical, emotional, and social dimensions of the person, and also taking into account the needs of the family and caregivers.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bff0bde elementor-widget elementor-widget-heading\" data-id=\"bff0bde\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Who is it aimed at?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d484dfa elementor-widget elementor-widget-text-editor\" data-id=\"d484dfa\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>The project is aimed at <strong>people included in the Integrated Care Process of Palliative Care of Sacyl<\/strong> who present:<\/p><ul><li>Incurable, advanced, and progressive disease.<\/li><li>Limited life prognosis, with little response to specific treatments.<\/li><li>Evolution with frequent crises and high demand for resources.<\/li><li>Great emotional and family impact that affects the caregiving environment.<\/li><\/ul><p>Since its launch, <strong>nearly 1000 people have been treated in the provinces of Valladolid, Zamora, Segovia, and Palencia<\/strong> <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f4cb9f3 elementor-widget elementor-widget-heading\" data-id=\"f4cb9f3\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">What services and support does it offer?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-052da3e elementor-widget elementor-widget-text-editor\" data-id=\"052da3e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul><li><p>The program combines a comprehensive care model with practical support that allows the person to remain in their home, in conditions of quality and safety. The main actions are: <\/p><ul><li><strong>Training and support for caregivers<\/strong>: training and accompaniment for professionals and non-professional caregivers to improve care and alleviate the family burden.<\/li><li><strong>Social and community participation<\/strong>: development of activities that allow maintaining bonds, relationships, and what is important for each person in their environment.<\/li><li><strong>Choice and guidance of the life project<\/strong>: the person defines, with the appropriate support, how they want to live this process and what they need to do it in their own home.<\/li><li><strong>Personalized home care<\/strong>: support for daily activities, focused on quality of life and staying at home.<\/li><li><strong>Prevention and socio-health coordination<\/strong>: comprehensive response to physical, psycho-emotional, and social needs, thanks to the joint work of social and health services.<\/li><li><strong>Home assessment and technical support<\/strong>: analysis of accessibility and provision of adaptations and technical aids that facilitate daily life.<\/li><li><strong>Personal and health care<\/strong>: support adjusted to each situation, including scheduled care.<\/li><li><strong>Accompanying in the final stage<\/strong>: positive support and active presence for both the person and their family, throughout the process.<\/li><li><strong>Support in grief<\/strong>: emotional and social accompaniment to the person&#8217;s environment after the
