It is about offering a care model that combines professional care with absolute respect for each person&#8217;s decisions and preferences, promoting their autonomy and quality of life. <\/p><p>The project is developed under a <strong>person-centered care<\/strong> approach, in which the participants themselves determine how they wish to receive support. community.<\/li><li>Prevention and maintenance of autonomy: proactive interventions to prevent the loss of capacities and promote independent living.<\/li><li>Personalized support plans: development and monitoring of life projects that reflect what is important to each person.<\/li><li>Comprehensive coordination: joint work between social services, health services, and third sector entities to offer coherent and effective care.<\/li><li>Adaptation of the environment: assessment of the accessibility of the home, carrying out adaptations, and provision of technical aids.<\/li><li>Technology and telecare: basic and advanced telecare services to reinforce security and accompaniment.<\/li><li>Personal and health care: support adjusted to the intensity that the person needs, including scheduled and emergency health care at home.<\/li><li>Training and support for caregivers: training and accompaniment for professionals, family members, and non-professional caregivers.<\/li><li>Social participation programs: activities designed with each person so that they remain linked to what matters to them and maintains their life and Third Sector entities dedicated to the care of people with disabilities and in situations of dependency, among them <strong>Fundaci\u00f3n
