dependency, disability or chronic illness the support they need so they can still live at home, life their life as normal, carry out day-to-day activities, become involved in the community, health care and, broadly speaking, any activities that make their lives more meaningful.<\/p>\n<p>Acknowledging the person\u2019s dignity, trying to make them as autonomous as possible, respecting their preferences and wishes, the capacity to adapt the support provided to the needs that may arise at any given moment and to help them become involved in the community are the aspects at the heart of any
