Description
Professional who will join a rural care team, as the first point of contact with people in a situation of dependency, providing information, guidance and advice on the range of services offered as well as managing new clients.
Duties
- Initial assessment of dependent people, identifying what the person needs and provide the necessary support.
- Draw up the support plan and the financial assessment thereof.
- Coordinate with professionals from the local public health and social services systems.
- Monitoring the work of personal assistants.
- Supervising and monitoring each case.
- .Participate in working groups
- Foster a sense of belonging to the community in coordination with local representatives.

Requirements
Hold a university degree in the field of socio-healthcare and/or complementary services; Social Work, Social Education, DUE, Occupational Therapy…
Professionals from the local area preferred.
